This replication process makes us prone to developing cancer cells in some cases. Now for a little techy stuff. Each strand of DNA has 4 letters in its code.  A, G, C and T and when a cell replicates, it may not make an exact copy.

Think of it like you are making copies on a copy machine, and each time you use the previous copy to make a new copy. Eventually the copy you are currently making is all fuzzy because the ink is not as full as it should be and the copies get hard to read. MESKIPUN garisan penuaan sudah terpancar di wajahnya, lelaki Cina berusia 80 tahun ini ternyata berbangga dengan dirinya sendiri. Otot tubuh Lawrence Cho kelihatan masih pejal dan berketak. Baru-baru ini, Cho yang berasal dari Batu Pahat, Johor, memenangi tempat ketiga bagi kategori Super Master dalam Kejohanan Bina Badan Kuala Lumpur 2012. Kategori bagi peserta berusi 55 tahun ke atas itu dimenangi oleh Mohamad Razali yang berusia 56 tahun manakala peserta berusia 65 tahun, Henry Lim menduduki tempat kedua.

Bertemu Cho di Super Body Fitness Centre iaitu gimnasium miliknya yang terletak di Jalan Klang Lama, Kuala Lumpur, terbukti ada sisi berbeza mengenai datuk kepada 11 orang cucu ini. "Saya tidak suka makan makanan segera dan mahu kekal aktif dalam sukan bina badan walaupun sedar saya semakin tua," ujar Cho yang mudah tersenyum.

Menurut Cho, beliau menceburi bidang tersebut semasa berusia 55 tahun iaitu ketika ramai rakan sebayanya yang memilih untuk menghabiskan masa berehat di rumah sahaja. "Sebenarnya saya terpengaruh dengan rakan saya yang juga ahli sukan bina badan bernama Sunny Phua. Dia pernah memenangi tempat ketiga dalam satu pertandingan bina badan pada tahun 1963.

"Phua banyak mendorong saya supaya terlibat dalam sukan ini," cerita Cho. Sekalipun rambutnya mula gugur dan dipenuhi uban, Cho masih tegap dan kuat. Melihat penulis yang agak kepenatan selepas menaiki tangga untuk menuju ke premisnya, Cho lantas menegur: "Hai orang muda, awak perlu lebih bertenaga dan kuat." Teguran itu membuatkan penulis berasa malu kerana usia penulis 50 tahun lebih muda daripadanya.

Cho memberitahu, sejak kecil beliau sudah obses dengan sukan terutamanya badminton, renang dan olahraga. Bekas pegawai sebuah bank tempatan ini menceritakan, selepas bersara daripada kerjaya formal itu, sukan bina badan adalah yang paling sesuai untuknya. "Jika main bola sepak saya perlu berlari dan melibatkan banyak pergerakan fizikal. Bina badan hanya memerlukan anda menjaga pemakanan selain latihan yang tidak ekstrem tetapi kerap.

Bina badan. "Malah, anda tidak perlu menghabiskan masa yang lama di gimnasium, cukup sekadar yang mampu. Lagipun saya memiliki pusat kecergasan sendiri," beritahu Cho. Pencapaian 'lelaki badang' ini boleh dibanggakan kerana beliau pernah menjadi juara Mr. Kuala Lumpur 2001 bagi kategori melebihi 45 tahun. "Saya banyak menyertai pertandingan tetapi kemenangan bukan matlamat utama. Saya cuma ingin menjaga kesihatan selain berkenalan dengan orang lain. "Tidak rugi bersahabat dengan ramai orang. Banyak pengalaman dan pengajaran yang kita peroleh. Apa sahaja yang kita lakukan, fikirkanlah sudut positifnya," ujar beliau.

Cho juga memberitahu, beliau sedih melihat rakan-rakan seusia yang mula mendapat penyakit selain meninggalkannya terlebih dulu. "Kematian itu satu yang pasti tetapi lebih baik apabila pada usia tua, badan kita masih sihat serta tidak menyusahkan orang lain. Malangnya, ada rakan yang tidak mampu berjalan dan sering sakit-sakit. "Saya tersentuh apabila seorang rakan memberitahu, dia banyak wang tetapi semua itu tidak berguna kerana hidup terlantar di rumah sahaja," ujarnya. Cho mendakwa beliau tidak menghidap penyakit kronik. "Demam dan selesema itu biasalah, saya bukan Tuhan dan tidak kebal. Pergi klinik, makan ubat kemudian pulih seperti biasa," cerita Cho.

Beliau yang gemar memakai seluar jean dan baju-T berkata, beliau melakukan latihan setiap minggu iaitu selama dua hingga tiga jam untuk setiap sesi. Bagaimanapun, sebahagian besar masa hujung minggunya dihabiskan bersama cucu dan anaknya. Penulis sempat meminta satu perkara pelik untuk dilakukan Cho iaitu mengangkat cucunya, Danny Cho yang berusia 27 tahun. Berat badan Danny ialah 65 kilogram dengan ketinggian 160 sentimeter, hampir sama dengan tubuh datuknya Cho. Ternyata Cho mampu mendukung Danny selama lebih 10 minit!

Ketika ditanya soalan sama ada ramaikah wanita muda yang terpikat dengan badannya yang tegap itu, Cho tersenyum. "Saya sudah tua dan sayangkan mendiang isteri. Bukan tidak ada yang cuba memikat. Malah ada daripada banyak kaum termasuk wanita Eropah. "Ada pula yang asyik menelefon dan menghantar SMS. Tetapi cukuplah dengan kehidupan sedia ada. Jangan rosakkan untuk kehendak yang tidak mendatangkan kebaikan," tutur Cho.

Pak Long genap seabad

BUKIT MERTAJAM: Seorang warga emas dari Kampung Tanah Liat Mukim 8 di sini, dianggap bertuah apabila masih dipanjangkan umur untuk meraikan ulang tahun kelahiran ke-100 pada tarikh keramat, semalam (12.12.2012). Dilahirkan 12 Disember 1912, Othman Daud mesra disapa Pak Long, masih sihat dan tidak menghidap sebarang penyakit, malah ingatan dan penglihatannya masih jelas. Buktinya, dia masih mampu membaca dengan baik tanpa menggunakan cermin mata dan boleh menulis dalam tulisan Jawi. Othman berkata, dia tiada sebarang petua khusus tetapi setiap hari bangun seawal jam 6.30 pagi dan memulakan rutin harian mengayuh basikal tua sejauh satu kilometer dari rumah untuk ke sawah diusahakan sejak zaman muda. Katanya, dia pulang sekitar jam 11 pagi untuk makan tengah hari dan keluar semula ke sawah jam 3 petang. “Pak Long tiada petua tapi sejak muda lagi suka makan pucuk ulam maman yang dibuat kerabu dan minum kopi ‘O’ serta air suam. “Alhamdulillah, mungkin amalan ini membuatkan Pak Long kekal sihat dan dengan izin Allah dipanjangkan usia hingga ke hari ini,” katanya pada majlis tahlil meraikan ulang tahun kelahirannya di kediamannya, semalam.

Anak sulung 12 beradik hasil perkongsian hidup pasangan Aishah Talib dan Daud Hasan yang sudah meninggal dunia itu kini tinggal bersama anak saudaranya, Faridah Ariffin, 44, yang juga ibu tunggal dan bekerja sebagai operator pengeluaran selain cucu saudaranya yang berusia 10 tahun. Manakala, tiga saudaranya iaitu Mah, 97, Bedah, 71, dan Romlah, 75, berkahwin dan tinggal berasingan. Meskipun sudah mencecah seabad, tidak sesekali terlintas baginya berumahtangga kerana takut melihat ramai berkahwin sering bergaduh hingga ada yang bercerai.

Study: Exercise Slows Muscle Wasting from Age, Heart Failure

Exercise can counteract muscle breakdown, increase strength, and reduce inflammation caused by aging and heart failure , according to new research in Circulation, an American Heart Association journal.

The benefits for heart failure patients are similar to those for anyone who exercises: there’s less muscle-wasting, and their bodies become conditioned to handle more exercise. The age of the patients didn’t matter, either, researchers found. “Many physicians—and insurance companies—still believe that cardiac rehabilitation does not really help in old age. This study clearly falsifies this belief,” said Stephan Gielen, M.D., lead co-author and Deputy Director of Cardiology at the University Hospital, Martin-Luther-University of Halle, Germany.

Between 2005 and 2008, researchers recruited 60 heart-failure patients and 60 healthy volunteers. Half of each group was 55 years and younger, and the other half was 65 years and older, resulting in an average age difference of 20 years between the groups. Half the participants in each age group were randomly assigned to four weeks of supervised aerobic training or no exercise.

Researchers took muscle biopsies of all participants before and after the intervention.

In both age groups, four training sessions of 20 minutes of aerobic exercise per day, five days a week plus one 60 minute group exercise session was associated with increased muscle force endurance and oxygen uptake. Heart failure patients 55 and under increased their peak oxygen uptake by 25 percent, while those 65 and over increased it by 27 percent.

Using biopsy results, researchers found that levels of a muscle protein indicating muscle breakdown, known as MuRF1, were higher in participants with heart failure than in their healthier counterparts. However, exercise reduced MuRF1 and reduced muscle inflammation, measured by levels of a protein called TNF-alpha. The strength of participants’ leg muscles was measured before and after the exercise. Younger and older heart failure patients increased muscle strength after the four-week exercise regimen. Muscle size was unaffected.

These findings offer a possible treatment to the muscle breakdown and wasting associated with heart failure and suggest that exercise is therapeutic even in elderly heart failure patients. The findings also suggest an avenue for drug development to slow muscle breakdown in heart failure patients.

“Exercise switches off the muscle-wasting pathways and switches on pathways involved in muscle growth, counteracting muscle loss and exercise intolerance in heart failure patients,” Gielen said. According to the American Heart Association, about 5,700,000 Americans age 20 and older have heart failure. “Over the last three decades, hospital admissions for heart failure have increased fourfold and will continue to do so, due chiefly to the aging of the population,” Gielen said. Estimates of costs vary, but are in the tens of billions of dollars per year in the United States alone, researchers said.

Tanggal 2006 adalah tahun paling mencemaskan kepada saya kerana didiagnos sebagai pesakit kanser. Tetapi saya bersyukur dan tidak panik walaupun penyakit itu kritikal.

"Saya yakin selagi mana saya ditakdirkan hidup, saya mantapkan fikiran yang positif untuk menghadapi segala kemungkinan,'' ujar Datin Rahmah Mahmood ketika berkongsi pengalaman mengenai kanser payu dara yang menyerang dirinya.

Cerita adik kepada Allahyarham Datin Seri Endon Mahmood, isteri kepada bekas Perdana Menteri, Tun Abdullah Ahmad Badawi ini, selepas pemergian kakak tersayang, dia disyorkan oleh doktor supaya menjalani ujian penentuan untuk memastikan bebas dari virus kanser.

"Setelah menjalani ujian saya disahkan sebagai pesakit kanser. Masa itu, hanya Tuhan yang tahu perasaan saya selepas disahkan ada virus itu di tubuh saya.

"Tambahan pula, saya sendiri menyaksikan keperitan melihat kakak Endon berhempas pulas melawan penyakit yang cukup memeritkan itu,'' ujarnya tenang mengimbau pengalaman dirinya disahkan menghidap kanser payu dara.

Katanya lagi, mengingatkan keadaan bagaimana kakaknya melawan penyakit itu, menjadikannya serik dan takut. Namun mengenangkan kenapa perlu mengalah dan menyerah sahaja pada takdir membuatkan wanita cekal ini berubah hati untuk bingkas bangun dan bertekad untuk melawan penyakit tersebut. "Saat itu juga saya mulakan misi untuk mencari rujukan dan maklumat. Malah ketika itu buku dan Internet adalah kawan yang paling rapat selain sokongan anak-anak.

"Malah hampir keseluruhan masa digunakan untuk mencari maklumat tentang penyakit tersebut.

"Sebenarnya bukan sesuatu yang senang untuk merawat kanser, saya perlu mengubah keseluruhan gaya hidup saya,'' ujarnya yang disarankan supaya menjalani pembedahan membuang ketumbuhan dan seterusnya menjalani proses kimoterapi.

Dalam usaha bersungguh-sungguh melawan penyakit, dia bersyukur seorang teman mengesyorkan dia bertemu dengan seorang doktor perubatan yang mempunyai klinik terapi di Melbourne, Australia iaitu Dr. Vagif Soultanov.

Tutur wanita cekal itu, kehadiran lelaki itu bagaikan penyelamat dalam hidupnya. Ketika itu, hatinya kembali berbunga.

Malah, kata-kata semangat daripada doktor terebut membuatkan dia seolah-olah menerima motivasi dan bersemangat untuk terus berjuang melawan penyakit nombor satu yang menyerang kaum Hawa itu.

Tambahnya lagi, ketika itu dia dinasihatkan oleh Dr. Soultanov supaya terus mengikuti prosedur menjalani pembedahan sebelum membuat rawatan herba.

"Mungkin berkat kesabaran dan kesungguhan diri, akhirnya saya berjaya menjalani pembedahan itu dan kemudian mencuba rawatan alternatif yang menggunakan bahan semula jadi,'' katanya pada satu majlis memperkenalkan ubatan herba semula jadi secara alternatif untuk rawatan pesakit kanser.

Datin Rahmah memberitahu, selepas menggunakan rawatan herba yang disyorkan oleh Dr. Soultanov keadaan dirinya semakin berubah dan menjadi semakin sihat dan kuat. Kini selepas sekian lama menggunakan rawatan alternatif daripada Dr. Soultanof dirinya disahkan bebas daripada kanser.

TIADA kata yang mampu diucapkan mana-mana wanita apabila disahkan menghidap penyakit kanser melainkan gambaran kematian yang terlintas di fikiran. Pemikiran mereka seolah-olah sudah ditanam dengan kepercayaan bahawa sesiapa sahaja yang menghidap kanser tidak mempunyai usia yang panjang. "Persepsi itu sebenarnya salah kerana Allah sudah menjanjikan yang terbaik buat kita sekiranya kita berusaha." Itulah bicara awal Rohana Rabu @ Mahat, 36, ketika berkongsi pengalamannya menghidap kanser ovari. Pada awalnya, Rohana sendiri berat hati dan malu untuk bercerita. Namun, katanya, dia berharap kisah ini dapat memberi semangat kepada pesakit lain melawan kanser. "Dengan izin Allah, keadaan kesihatan saya semakin baik. Mula-mula dulu saya seperti wanita lain, sedih, malu dan hampir berputus asa. Namun, saya yakin dengan ketentuan Tuhan, setiap penyakit pasti ada ubatnya. Saya berusaha berjumpa pakar dan mencari rawatan alternatif," ujarnya yang bertugas sebagai guru. Empat tahun lalu, Rohana disahkan menghidap kanser ovari peringkat ketiga. Seperti tidak percaya dengan berita itu, Rohana cuba bangkit dan mencari semangat baru dalam hidupnya.

Ujian

Selepas menjalani ujian pemeriksaan kesihatan, Rohana perlu melakukan pembedahan segera. Rohana bersyukur kerana segalanya berjalan lancar dan seperti pesakit kanser lain, dia kemudian menjalani rawatan kemoterapi. "Tuhan sahaja tahu sakitnya ketika itu. Namun saya positifkan diri untuk melawan sakit. Saya berpantang dari segi penjagaan makanan dan hanya bergantung pada ubat selama empat tahun. Kemudian suatu hari, saya mencuba suplemen herba setelah disarankan oleh seorang kawan. Dengan izin Allah, kesihatan saya semakin baik terutamanya aspek kecergasan fizikal. Walaupun begitu, saya tetap berpantang makan kerana takut rahim menjadi bengkak semula," ujarnya ketika ditemui di An Nabaat Sdn. Bhd, Bandar Baru Bangi, Selangor baru-baru ini. Tambah Rohana, dia dimaklumkan faktor pemakanan dan genetik menjadi punca terbesar kanser ovari. Justeru, Rohana lebih berhati-hati soal pemakanan. Nasihatnya, walau sibuk dengan kerjaya, setiap orang perlu elakkan makanan segera dan banyakkan makanan organik atau sihat seperti sayur-sayuran dan buah-buahan. "Kehidupan hari ini berbeza dengan dulu kerana semua perlu dilakukan segera. Masa pun berjalan dengan pantas. Oleh itu, kita perlu bijak mengurus masa dan jadual diri supaya kita juga mempunyai masa berehat yang sempurna. Selain itu, kita perlu kembali kepada amalan semula jadi sebagai umat Islam. Untuk menjadi positif, banyakkan amal ibadat seperti solat sunat dan membaca al-Quran. Itu semua akan menjadi penawar terbaik kepada setiap pesakit," katanya mengakhiri bicara.

Cancer in Africa: Fighting a nameless enemy

ACCRA (Reuters) - In Emanuel Adu's language, Twi, people call the skin cancer that is invading his cheek and nose "sasabro". It means a disease that eats away at you. The 73-year-old former cocoa farmer has come to the Korle Bu Teaching Hospital in Accra, miles from his home, to be treated with one of the two radiotherapy machines in Ghana. "I had heavy bleeding and discharge from my nose. The doctor told me it was cancer, a cancer called melanoma, but I had not heard those words before," he explains in the consulting room. Most of Africa's around 2,000 languages have no word for cancer. The common perception in both developing and developed countries is that it's a disease of the wealthy world, where high-fat, processed-food diets, alcohol, smoking and sedentary lifestyles fuel tumour growth. Yet Adu's is one of an estimated one million new cancer cases sub-Saharan Africa will see this year - a number predicted to double to 2 million a year in the next decade. How can a continent hope to diagnose and treat, let alone fight to prevent a disease that has no name?

 

CANCER KILLS THE POOR TOO

It's a question David Kerr has been struggling with for several years. A cancer specialist based at Britain's Oxford University and former president of the European Society of Medical Oncology, Kerr set up the charity AfrOx in 2007 to help African countries seek to prevent and control cancer. "There have been some marvellously effective campaigns around AIDS, tuberculosis (TB) and malaria, and of course infectious diseases like those are terribly important," he said in an interview. "But already there are more deaths in the world from cancer than from AIDS, TB and malaria combined." By 2030, according to predictions from the World Health Organisation (WHO), 70 percent of the world's cancer burden will be in poor countries, a prediction Kerr says leaves most lay people, and even many doctors, "utterly astonished". "They think cancer is a disease of the wealthy. But the reality is that, in part because of success in tackling infectious diseases, Africans are living longer. It's almost a booby prize that they're now living long enough to get cancer." For many women in Africa, that means diseases like breast and cervical cancer have become common causes of death before their victims have begun to learn about them, let alone find words for them. A study published in 2011 found that since 1980 new cervical cancer case numbers and deaths have dropped substantially in rich countries, but increased dramatically in Africa and other poor regions. Overall, 76 percent of new cervical cancer cases are in developing regions, and sub-Saharan Africa already has 22 percent of all cervical cancer cases worldwide. The same research found that some poorer countries saw a rise in breast cancer cases of more than 7.5 percent a year over the 30 year period studied - more than twice the global rate.

 

"EXPLOSION OF CANCER"

Such data come as no surprise to Verna Vanderpuye, a clinical oncologist and radiotherapy consultant at the Korle Bu hospital where Adu will come for his treatment every day for the next two weeks. "There's an explosion of cancer here," she said in an interview in her overrun consulting room. Unlike the diseases she is trained to treat, the oncologist is a rare thing in Ghana. She is one of only six trained cancer doctors who serve a country of 24 million people. Good quality data are hard to come by, but Vanderpuye says the two main hospitals equipped to treat cancer - hers and another further north in Ghana's second city of Kumasi - are seeing between 5,000 and 7,000 new cancer patients a year. "That's just the tip of the iceberg. That's only those who actually come to the health facilities. We don't really know what's really happening across the country," she says. It seems a drastic situation. But Ghana has come a long way. Until 2007, there were no oncologists at all - and that's still the case in some neighbouring countries. Sierra Leone, for example, has more than 6 million people and no cancer doctors. "Countries in the region like Sierra Leone, Togo, Guinea have absolutely nothing in terms of cancer care," said Kerr. "That means Ghana's six or so oncologists have to serve neighbouring countries as well." The result is that the reality for many in Africa is that a cancer diagnosis means a painful and distressing death. The overwhelming number of cases and the paucity of funds, doctors and treatment mean it's difficult to know where to start, Kerr says. But cancer experts - foreign and African - and patients and advocacy groups say what's needed first is greater awareness.

 

KNOWING THE ENEMY

Ellen Awuah-Darko is doing what she can. The 75-year-old founder of the Accra-based Jead Foundation for breast cancer says her own experience - of finding a lump and ending up paying tens of thousands of dollars to be treated in the United States - made her to try force change. "In America I had to put down $70,000 (43,217 pounds) before they'd even talk to me," she said. "I was lucky, I could afford it after my husband died and left me money, but I thought 'why should I get treatment when others can't'." Now, every Wednesday, Awuah-Darko goes with healthcare workers into communities in the Eastern Region of Ghana to offer women breast screening. It's not the high-tech mammogram or ultrasound scan women in wealthy countries are used to, but a simple breast examination and a lesson in how to self-check. "Early detection can save your life. I want everybody to know that. It's not something people should be ashamed of or embarrassed about," she said. But she and the handful of cancer specialists are fighting deep cultural resistance - not only to the idea that cancer affects people here, but also to the idea they must talk about it, look for it and recognise it to start fighting against it. Even among the young and educated, cancer is often taboo. "They don't want to use the C-word," says Vanderpuye. "That's also one of the main reasons why someone wouldn't want to come here - because it means she has รข€˜the C'.

 

"HUMONGOUS FOUL - SMELLING TUMOURS"

In the chemotherapy ward at Korle Bu, oncology nurse Juliana Tagoe, explains why patients often don't want to talk about cancer. Many people see the disease as a spiritual punishment, she says. "They think someone has done wrong and this is the effect - God is punishing them. They feel stigmatised." In rural communities where spiritual and tribal leaders are revered, the use of prayer, ritual and herbal remedies is common. Awuah-Darko says witch doctors tell patients with tumours to "treat it like a boil, and just put some herbs on it". Others are told simply to pray for it to be taken away. In the months or years that intervene, the tumours spread and grow to sizes barely seen by doctors in developed countries. Kerr talks of patients in Africa with tumours that protrude through the breast or encircle the whole chest, while Vanderpuye describes patients with "humongous, foul-smelling tumours" she has little hope of treating.

 

INFECTIONS

Both Kerr and Corey Casper, who runs the Uganda Programme on Cancer and Infectious Disease associated with the Fred Hutchinson Cancer Research Center in Seattle, say another focus of efforts to tackle this looming cancer epidemic is to try to prevent the cancers caused by infections. While many cancers are linked to lifestyle factors such as unhealthy diets and smoking, a large number - particularly in Africa - are caused by infections likes hepatitis B and C, which cause liver cancer, and the human papillomavirus (HPV) that causes almost all cervical cancers.

In wealthy countries, having hepatitis vaccines as part of routine childhood immunisation programmes, and introducing national campaigns programmes with new HPV vaccines from drugmakers Merck and GlaxoSmithKline has brought rates of liver and cervical cancer down significantly. If such nationwide HPV vaccine campaigns could be introduced in Africa, experts say, the effect on rates of death and disease could be dramatic. Global health groups are working with drugmakers on securing a discounted price for HPV shots for poor countries, but getting them to Africa could take years. Akosua - a name meaning "Sunday" that this patient gives instead of her real name - has no concept yet of how an injection made by a Western pharmaceutical company could have prevented the cancer spreading inside her. The 48-year-old farmer has cervical cancer and has suffered with a lot of pain and bleeding, but for now the fact that she's come to the hospital and is seeing an oncologist is foreign territory enough. "But I'm not afraid," she says. "I've been seen by the doctor now. I know I am in the right place to get the right treatment."
